Intended use:
Publication on the Physioplus+ Website, Facebook Pages and all Social Media for pictorial representation of the Photographed Person for the duration of the working relationship with Physioplus+.
 
Declaration:
The undersigned declares his/her consent with the (unremunerated) use of the photographic images of his/her person for the purposes described above. Use of the photographic images for purposes other than those described above or for marketing via the transfer of the images to third parties is prohibited.
